BI-MONTHLY STATISTICAL REPORTING
Please select your agency name from the list below to download the statistic reporting form. Select "SAVE" to save the form to your computer. This can be done each reporting period if needed. Unless informed of a longer time being allowed, reports are due by the 10th of odd months for the prior two months of statistics.
To work within the form, tab through the highlighted areas to enter your information. The 'number seen first time this fiscal year' is based on Mental Health & Mental Retardation Services' fiscal year, July 1, 2009 through June 30, 2010. For your first month reported in each fiscal year, all clients that you have seen within the reporting period are considered 'seen for the first time this FY'. After the first report, enter only those who you are actually seeing for the first time. However, on all reports, enter all Units of Service (as defined by your agency) that you have provided--including those who are not being seen for the first time.
